Position

Full-time, Case Manager/Rehabilitation Consultant with experience in providing community based services to a variety of clients including, but not limited to individuals with acquired brain injury, spinal cord injury, multiple orthopaedic injuries, chronic disease and/or stroke.

Regulated Health Care Professional
(Registered Nurse, Occupational Therapist, Social Worker)

Member in good standing of governing College

Ability to work independently in community setting and collaboratively on trans-disciplinary health care teams

Strong organizational and oral/written communication skills

Excellent interpersonal skills

Valid driver’s licence

Assets

5 years of clinical experience
CRRN and/or CCRC designation
Clinical experience with individuals with acquired brain injury,
spinal cord injury, and multiple orthopaedic injuries
Experience in auto and work place insurance industries
Bilingualism
