Pain Management program
The TRAC Group’s highly skilled bilingual team includes Registered Nurses, Speech-Language Pathologists, Communicative Disorders Assistants, Occupational Therapists, Life Skills Workers, Vocational Rehabilitation Counselors and Rehabilitation Consultants. Working together, we design and deliver services that are tailored to each client’s unique needs.
Our approach
Our goal is to attain medical stability through improving the client’s experience of pain, level of activity and overall functioning in preparation for vocational and vocational activities. The pain management program is executed under the direction of our Program Coordinator who is a Registered Nurse, certified in Pain Management with the Ontario Nurses Association. (ONA). The Program Coordinator is first point of contact for clients and is a pivotal member of our treating team comprised of an Occupational Therapist, and Cognitive Behavioural Therapist in addition to the Program Coordinator and all of whom have Progressive Goal Attainment Program (PGAP) certification.
Program strategies include:
- Completion of a pain assessment using an interview format and standardized assessment tools
- Provision of education
- Communication with treating physician
- Client monitoring with respect to medication compliance and associated benefits
- Referrals to/completion of services in order to assist with integration into activities. Services include occupational therapy, cognitive behavioural therapy, RA and vocational activities.
- Completion of the Progressive Goal Attainment Program
Every TRAC Group client’s file is reviewed by either our Clinical Director, Vocational Manager or Discipline Specific Practice Leader, with a minimum of 12 years’ experience in community rehabilitation. This review ensures best practice guidelines and program protocols are followed and rehabilitation plans are working as they should be.
