Future Cost of Care Report
The TRAC Group’s highly skilled bilingual team includes Registered Nurses, Speech-Language Pathologists, Communicative Disorders Assistants, Occupational Therapists, Life Skills Workers, Vocational Rehabilitation Counselors and Rehabilitation Consultants. Working together, we design and deliver services that are tailored to each client’s unique needs.
Our approach
Life care planning is a multidisciplinary, objective process to identify an individual’s current and future health requirements, relative to the specific injuries and impairments sustained by a client arising from complex or catastrophic injuries or from a chronic illness. The outcomes of this process, inclusive of current and future health requirements and associated costs are documented in a Life Care Plan or Future Cost of Care Report.
Executed under the direction of Ms. Ruth Leppard, Certified Rehabilitation Registered Nurse and Certified Life Care Planner, our goals are to enable clients to maintain their medical stability, independence and maximum level of function, through the development of an objective life care plan.
Based upon research, analysis of medical and health-related professional records, client meetings and health care professionals’ recommendations, we develop a Future Cost of Care Report. The report, utilizing a template developed by TRAC, documents in an organized and concise fashion, the current and future health requirements, options and associated costs relative to the specific injuries and impairments sustained by a client arising from complex or catastrophic injuries or a chronic illness.
Services include:
- Analysis and review of clients injuries, impairments and rehabilitation progress/treatment to date
- Meeting with client to obtain his/her insights
- Communication with treating professionals to obtain recommendations relative to future requirements vis a vis client’s injuries, impairments and rehabilitation progress/treatment to date
- Completion of research further to client requirements, professional recommendations, and treatment options and associated costs
- Development of a life care plan
