Rehabilitation Consulting Services

The TRAC Group’s highly skilled team includes Registered Nurses, Speech-Language Pathologists, Communicative Disorders Assistants, Occupational Therapists, Life Skills Workers, Vocational Rehabilitation Counselors and Rehabilitation Consultants. Working together, we design and deliver services that are tailored to each client’s unique needs.

Our approach

Provision of an organized framework for effective and proactive rehabilitation consulting services through:

• Assessment and evaluation of all aspects of client injury/disability. This includes medical, vocational, psychological, social and behavioural factors.
• Development of cohesive, goal orientated rehabilitation plans.
• Coordination and implementation of recommendations/requirements for disability management in keeping with individual client needs.

The following services are available on a stand-alone basis or in combination as per the required needs.

• Rehabilitation Needs Analysis
• Consultation with physicians to secure medical status reports and treatment recommendations
• Identification and development of individualized and cost effective rehabilitation treatment plans
• Assessment of Normal Life Activities
• Assessment of Attendant Care Needs
• Coordination of Independent Medical Evaluations (IME)
• File review
• Needs analysis

Every TRAC Group client’s file is reviewed by either our Clinical Director, Vocational Manager or Discipline Specific Practice Leader, with a minimum of 12 years’ experience in community rehabilitation. This review ensures best practice guidelines and program protocols are followed and rehabilitation plans are working as they should be.
