If you or a family member are coping with complex injuries
or impairment, you may be feeling overwhelmed and have many
questions.
To what extent will you or your loved one recover? What happens
after discharge from the hospital or other in-patient facility?
What rehabilitation services are needed to maximize recovery?
How do you access them? How long is the wait? Is your house
safe and accessible? Will it be possible to resume work and
other activities?
If the injuries are the result of an accident, you may also
be dealing with insurers and lawyers, who require objective,
accurate and timely information about the rehabilitation process,
in order to pursue claims or authorize payments. We can help.
What we do
The TRAC Group provides a full
range of customized, bilingual rehabilitation programs to
adults and children across Eastern Ontario. We can start working
with you as soon as a diagnosis is made, helping with hospital
or in-patient facility discharge planning. We can:

Help you identify the rehabilitation programs and
services that will maximize independence and quality
of life |

Design and provide a comprehensive, customized, goal-oriented
and effective multi-disciplinary rehabilitation program,
including programs for those with traumatic brain injury,
spinal cord injury or who are recovering from stroke |

Provide services on a long-term basis or on an interim
basis while you or your family member are on a waiting
list |

Provide services in a package or singly |

Provide services at home, school, the workplace
or community setting |

Assess home safety and accessibility |

Identify appropriate wheelchairs, walkers, scooters
or other mobility devices and help you get them as soon
as possible |

Identify appropriate communication aids and help

Provide return-to-work and other vocational programs |

Help you determine whether publicly funded rehabilitation

Provide objective and timely assessments and evaluations

Additional support
As a client or family member, you will also be able to attend
our group programs, monthly educational seminars and have
access to our library of multimedia information resources.
We also encourage you to ask questions, so you understand
the recommended treatments and how they will help.
You don’t have to navigate the system on your own.
