We work with all types and severity of injuries. If you or a loved one has sustained a Catastrophic Injury, TRAC Group also offers a number of specialized programs customized for individuals with complex care needs.

Programs

We understand the physiological, psychological, vocational and social impact that traumatic brain injury has on the person who has suffered the injury. We also understand the impact on caregivers and family members.

Our goal is to help clients maximize their independence and quality of life at home, work and in their community. We use an interdisciplinary approach, meaning our therapists work together to develop and provide rehabilitation programs and services tailored to each client’s unique needs. Our integrated and comprehensive program reduces the number of transitions our clients are required to make as they progress through various stages of recovery which helps them to achieve and maintain the best possible outcomes.

We also educate clients and families about what they can expect during the rehabilitation and recovery process. In addition to providing evidence-based cognitive rehabilitation, we ensure each client’s equipment, attendant care, mobility and accessibility needs are met so they are free to focus upon their recovery and achieve the best possible quality of life.

Our traumatic brain injury program team includes Physiatrists/Physicians, Rehabilitation Consultants, Occupational Therapists, Registered Nurses, Speech-Language Pathologists, Physiotherapists, Psychologists, Cognitive Behavioural Therapists, Vocational Rehabilitation Consultants, and Rehabilitation Assistants. TRAC GROUP Inc.’s Pain Management & Functional Restoration Program is a unique, multi-disciplinary program that emphasizes pain management in the context of return to function. Be it household tasks, leisure activities, or return to work, clients are supported in reintegrating into their activities of daily life through the provision of multi-modality pain strategies and participation in the Back on TRAC Reactivation Program©, a goal-oriented, time-limited program designed to return clients to their optimal activity levels.

Staffed by Registered Nurses with expertise in pain assessment and management, Occupational Therapists, Physiotherapists and Behavioral Therapists, the TRAC Pain Management Team understands that the experience of pain is complex and can impact mood, cognitive functioning, social relationships, and quality of life. TRAC Pain Consultants are Registered Nurses with the training and experience to tailor a comprehensive pain-management plan that minimizes both the physical and emotional impacts of chronic pain while maximizing clients’ return to meaningful activities.

The Back on TRAC Reactivation Program© is a goal-oriented, time-limited program that guides clients through a graduated return to meaningful activity while assisting them to incorporate recommended strategies for pain management.  Unlike traditional pain management programs, clients are supported to achieve attainable, measurable reactivation goals that result in clients safely resuming activities of daily living while managing the experience of pain.

TRAC’s Healthy Transitions Program provides support, education, and resources for individuals and their families to enhance resiliency and strengthen families.

A brief, solution-focused intervention supporting clients and their families to independence through education. Facilitated by Registered Social Workers and Psychotherapists, this innovative program addresses common psychosocial reactions to trauma, provides practical coping strategies and community resources to support the recovery process to independence.

For many individuals transitioning home from the hospital or embarking upon the rehabilitation process following illness or injury, there can be gaps in services and information both of which put individuals at risk of developing maladaptive psychosocial reactions or coping strategies.

With shorter hospital stays, limited home supports, and increased expectations for care by families, often transitions from injury to rehabilitation, or hospital to home is fraught with stressors such as: loss, role changes, financial strain and family conflict. In these situations it is understandable that individuals and their families can become emotionally distressed, overwhelmed and have difficulty coping.

While psychosocial issues can be barriers for positive outcomes in the rehabilitation process, research suggests that studies have reported that early psychosocial intervention improves long term outcomes by promoting self-efficacy, stress reduction and improved family functioning. Additionally, such intervention can prevent crisis situations and lessen the burden on other rehabilitation services.

At The TRAC Group, we have identified a short-term approach that can help bridge the gap as clients and their families transition across the continuum of care to recovery and independence.

How the TRAC Healthy Transitions Program can help by:

  • Educating clients and their families on the psychosocial/ emotional aspects of illness, injury or disability.
  • Providing information about the community resources appropriate for the unique needs of individuals and their families.
  • Offering practical tools to cope with emotions such as fear, anxiety, sense of loss, sadness, and anger.
  • Providing brief, solution-focused counselling on issues of adjustment, loss, role changes, relationship issues, or other difficulties related to injury, illness or disability.

The TRAC Healthy Transitions Program is:

  • Time-limited – 3 sessions offered in-home or in-clinic during the day or evening hours.
  • Goal-oriented -education, resources and brief, solution-focused counselling.
  • Focused upon education, practical assistance and solution-focused counselling during the transition home or the beginning of the rehabilitation process.
  • Designed to address the psychosocial and family issues related to illness, injury and disability in an effort to improve functional outcomes.

After The TRAC Healthy Transitions Program:

It is anticipated that individuals and their families will be stronger and better able to cope with the challenges injury or illness can bring. Individuals and their families will be empowered to self-advocate and access the community resources they need, having been provided with the requisite tools.

For individuals demonstrating signs of persistent psycho-emotional distress after the program, their health care practitioners will be contacted to discuss an appropriate course of action.

Occupational Therapy promotes and enables people to participate in the activities which give meaning and purpose to their lives. TRAC Group Occupational Therapists work with clients experiencing physical, cognitive, and/or psychological obstacles to engage in and improve their function and independence across the spectrum of life, encompassing self- care, as well as home, work, and leisure pursuits.

Speech-Language Pathology is a discipline concerned with speech and language communication disorders and swallowing disorders. TRAC Group Speech-Language Pathologists work with clients to address cognitive communication challenges, inclusive of difficulties with concentration, memory, understanding language, reading, writing, reasoning, or verbal planning and organization.

TRAC Group Speech-Language Pathologists provide the following services:

Physiotherapy targets the identification and remediation of physical impairments and the associated disabilities while promoting mobility, functional ability, quality of life, and movement potential. Physiotherapy may include the prescription of or assistance with specific exercises, manual therapy, education, manipulation, and development of fitness and wellness-oriented programs for restoration and maintenance of function after a motor vehicle accident.

TRAC Group’s Rehabilitation Consultants are qualified allied health professionals employed to provide rehabilitation services to clients with a disability, injury or chronic health condition. Our Rehabilitation Consultants provide an organized framework for effective and proactive services through assessment and evaluation of all aspects of a client’s injury/disability; development of cohesive, goal oriented rehabilitation plan; and coordination and implementation of recommendations in keeping with individual client needs. At TRAC, our Rehabilitation Consultants hold one of the following designations: Registered Nurse, Occupational Therapist, Registered Social Worker, and Certified Disability Management Professional.

In-clinic-based psychological assessment and treatment services provided by a Registered Psychologist are available for both French and English speaking clients throughout Eastern Ontario. Treatment modalities include Cognitive Behavioural Therapy, Psychoanalysis, Motivational Interviewing, and Imagery Rehearsal Therapy for mental health conditions such as Depression, Anxiety, Trauma and Traumatic Brain Injury.
In-clinic psychotherapy services provided by Registered Psychotherapists in both French and English. Treatment modalities include Cognitive Behavioural Therapy, Motivational Interviewing, Addiction Therapy, Dialectical Behavioural Therapy and Imagery Rehearsal Therapy for mental health conditions such as Depression, Anxiety, Addiction, Trauma and Traumatic Brain Injury.
Social Workers are consulted when individuals are faced with a difficult period in their personal, family, and/or work life. TRAC Group Social Workers help identify the issues and their sources, assist individuals in developing appropriate coping skills and problem solving strategies, help individuals navigate available resources, and offer counselling in various forms.

Designed to reinforce desired behaviors and eliminate undesirable behaviors, behavioral therapy is a psychotherapeutic approach that may assist individuals following injury or illness to reduce or eliminate maladaptive coping strategies and establish healthy behaviors that promote successful and enduring recovery and quality of life.

TRAC Group employs regulated personnel who provide Rehabilitation Assistance to clients under the direction of our experienced, regulated health care professionals, such as Occupational Therapists, Physiotherapists, Speech-Language Pathologists, Registered Nurses, and Social Workers. Our Rehabilitation Assistants provide home- or community-based intervention sessions as an adjunct to therapy giving clients increased opportunity to realize their rehabilitation goals in a cost-effective manner.
