Traumatic Brain Injury (TBI) Program

At the TRAC Group we understand the physiological, psychological, cognitive, vocational and social impact that traumatic brain injury has on the person who has suffered the injury. We also understand the impact on caregivers and family members.

TRAC Group was a pioneer in establishing community based, multidisciplinary programs for individuals with catastrophic injuries such as traumatic brain injury. Adopting features from hospital rounds, Designated Assessment Centres (DACs) and traditional community care models, the TRAC TBI Program is an innovative delivery of care model that incorporates best practice guidelines, communication protocols and functional outcome measures with the ultimate goal of improving care and reducing costs for clients.

Our goal is to help clients maximize their independence and quality of life at home, work and in their community. We use an interdisciplinary approach, meaning our therapists work together to develop and provide rehabilitation programs and services tailored to each client’s unique needs. Our integrated and comprehensive program reduces the number of transitions our clients are required to make as they progress through various stages of recovery which helps them to achieve and maintain the best possible outcomes. We understand that clients with access to coordinated, inter-professional care have better outcomes when compared to services provided in isolation.

We also educate clients and families about what they can expect during the rehabilitation and recovery process. In addition to providing evidence-based cognitive rehabilitation, we ensure each client’s equipment, attendant care, mobility and accessibility needs are met so they are free to focus upon their recovery and achieve the best possible quality of life.

Staffed by Regulated Health Care Providers and Allied Professionals, TRAC’s breadth and depth of expertise enables our professionals to effect positive and enduring change in the lives of our clients.

TRAC is an abbreviation for Traumatic Rehabilitation @ Community
